Community Residential Program

Overview

There comes a time in everyone’s life, where they leave their family home to gain independence and freedom of living independently. For people with disabilities, this can happen at different ages and stages of life.

Our person-centered residential services support each person to live an interesting life, connected to the people and the things they love as independently as possible.

Our goal is to have each person we support direct their own lives, make their own choices and understand their rights and freedoms.

The 24-hour shift staffed home is customized to the people living there (usually three people share a home). Each person has their own bedroom and share the common spaces.

Adults 18 years and older

With a referral from:

  • Community Services Worker through Community Living disability Services (CLDS)

Q:

Is there a nurse or other healthcare professional in the home at all times?

While we consult with nurses and allied health care professionals, our 24-hour staff are not nurses. Most 24-hour staff are trained support workers. This applies for the Community Residential Program.

Q:

How much does this cost for my loved one?

When you’re accepted into this program, all shelter, food and staffing costs are funded. There is no cost to you for the above listed items.

Q:

How much will I remain involved in my loved ones life?

Yes! Your relationship will continue as it’s always been, you can be involved as much as you and your family member would like. We often see a varying a mount of involvement from different families, like when anyone moves out of their childhood home for the first time-some people go visit their childhood home once a week, others more frequently-and some come back only for special occasions and holidays.

When we receive a referral we determine whether there is a current opportunity to explore or whether we anticipate future opportunities

How do I get in touch with someone at CLDS to access your services?

If you have a Community Support Worker assigned to you (sometimes people call them a CSW) we encourage you to reach out to them.
