Community Stabilization Service
Overview
Working closely with case workers, agency staff and families, the Community Stabilization Service (CSS) supports people who are risk of a placement breakdown that could cause them to leave their home, day program or other important parts of their lives because of complex behavioural and health needs.We often support people who have a dual diagnosis of intellectual disability and complex mental health needs.
The CSS provides a short-term intervention from an interdisciplinary team that includes social work, nursing, psychology, psychiatry and pharmacy.Services generally take three months to complete.The CSS provides extensive supports to members of the person’s support network and helps with advocacy and systems navigation to make sure the person is receiving the right services over the long term.
The CSS also offers regular training to help families and direct support professionals learn more about supporting people experiencing challenges.
Who Qualifies?
Adults who are enrolled with CLdS.
With a referral from a Community Services Worker (CSW) through
- Community Living disAbility Services (CLdS)
